Start with a large white canvas, in your layers window make a new layer set and name it Backdrop Design. Create a new layer and draw a 4763 x 164 pixels, #BDBDBD colored rectangle.

With your rectangle layer from step one add the following layer styles:

In a new layer draw a 20 x 16 pixels, black rectangle on the top corner of your rectangle. Now cut the rectangle in slant like you see on the picture below.

Using your Zoom tool, zoom on the corner design and cut in a slant like you see on the picture below.

With your corner design layer add the following layer styles:

Now add the same corner design on the other corners of the rectangle. Make a marquee selection around the corner design then go to Edit > Copy Merged and back to Edit > Paste. Rotate the copy through Edit > Transform > Flip Horizontal.

Add the rest of the corner design and your done.
