[1] Take a new file of 500 pixels, 500 pixels, of resolution 72 dpi in the RGB mode.

[2] Take the Rounded Rectangle Tool use these settings.

[3] Create a shape.

[4] Now go to View>Rulers, Click on the top ruler and drag it into the image to create line. like this.

[5] Then take the Rectangular Marquee Tool use these settings.

[6] And now create a shape over the selection see in the next image.

[7] Your image should be as shown below.

[8] Now use the same think to the Bottom selection.

[9] Then make the new layer and fill the selection with Black Color.

[10] Then go to Edit>Transform>Skew.

[11] Go to Blending Option use the following settings.

[12] Your image should be look like this.

[13] Take the Pen Tool create a shape like this.

[14] Create a new layer make the selection and fill the selection with Black color.

[15] Go to Blending Option use the following settings.

[16] Your image should be look like this.

[17] Now open your wallpaper image.

[18] Then Cntl+Click on this layer see in the next image.

[19] Now select the wallpaper image layer then press Cntl+X, and press Cntl+V, to paste it now, delete or hide the wallaper image layer.

[20] Change the Blending to Multiply.

[21] Your image should be as shown below.

[22] Take the Elliptical Marquee Tool create a shape and fill with White color.

[23] Here is a final step change the Blending Mode and set the Opacity.
