Start off by making a new document (Mine is 250 * 600 But any vertical size will work.) and make a new layer, Select all of the document (Ctrl + A) and fill it with white. Right click on the layer in your layers palette and click blending options,
Apply these layer styles:

Your banner should look somewhat like mine now:
Now for the plans boxes First make a new group (Click the folder on layers palette) and name it Plan One
Now using your rounded rectangle tool, make a box for plan one with a radius of 10px (Top of the screen when the rounded rectangle tool is selected.)
This is what the box should be in size and position:
Now make another box, This time smaller and make sure it is under the box we made a few seconds ago in the layers palette It should be about like this:
Now that you have these boxes, We need to add layer styles to them Lets start with the first box we made (Larger box.)
Inner Glow:
Gradient Overlay:

Now for the smaller boxs styles:

Now this is what it should look like with the styles:
And with some text added (For the Plan One Text I used the same style as I did with the header text):
If yours does not look right, Make sure the smaller box is under the larger one in the layers palette.
Now that you have your plans box made and in a group altogether, Simple duplicate the group and move it down for the other boxes. (You can move the group all at once by the way.)
Here is what mine looks like with all the plans boxes.
I have also made another button (The button itself uses the same styles as the buttons on the plans,):
